Qualifying Patient Survey

On October 29, 2024, registered qualifying patients were sent an email from the Department requesting individuals to complete a survey. The survey will help the Department better understand how the Medical Marijuana Program is working. The survey is being conducted at the request of the Medical Marijuana Advisory Board. The survey will be available through November 26, 2024.

Respondents must include their medical marijuana card number (located on the front of the card) on the survey. Surveys completed without a valid card number will not be included in the analysis of results. The survey can be accessed by clicking here.

Registered qualifying patients with questions regarding the survey are asked to contact the Department at medmarijuana@nd.gov.
