Welcome to the Rural Health Transformation Program (RHTP) funding opportunities webpage. Here you will find information about the latest opportunities and learn how to apply.

We will be launching multiple funding opportunities as part of North Dakota’s five-year RHTP effort.

The funding is designed to support practical, locally driven solutions that help rural and tribal communities through four pillars:

  • Strengthen and Stabilize Rural Health Workforce
  • Make North Dakota Healthy Again - Preventive Care, Eating Healthy
  • Bring High-Quality Health Care Closer to Home
  • Connecting Technology, Data and Providers for a stronger ND

Funding opportunities will be released in phases, with individual applications announced over time.

Continue to monitor this webpage for updates.

New Vendors, Contractors, Providers

If you’re new to doing business with the state, there are a few required steps you’ll need to complete before contracting and receiving payment(s).

1. Register with the Secretary of State to perform business in the state.

2. If your organization has not been paid by a state entity before, you must register with vendor registry before you can be paid. Here’s a how-to guide on vendor registration.

3. ND HHS will require a W9 form to set up by your organization within our contract system. If your organization would like to be paid with electronic funds transfer, you need to submit the substitute IRS form W-9.

4. If your organization will receive funding under a federally funded program, you must register in SAM.gov and obtain a Unique Entity Identifier (UEI) prior to contract execution or payment. Your SAM.gov registration must remain active for the duration of the award.
