EMS Personnel / Agency Registry (Big Picture) - *Emails were sent to EMS personnel on 9/22/2023 explaining the new login process. Upon your initial access to the new Big Picture system you will need to create a new password. Please follow instructions located HERE. At this time personnel have the ability to update personal information and print their certificate/license. The ability to renew certifications/licensure is not available yet. - 2025 EMS Registration: Utilize this form for initial/renewal of North Dakota state licensure for BLS personnel, student registration, agency roster updates and changes to demographics. This form is also to be used by registered nurses seeking to provide care on a licensed ambulance service. Note: as of 2022, this form is TWO (2) pages in length - BOTH pages must be submitted for processing.
- Non-Nationally Registered ND EMR Continuing Education Report: This form is to be used for re-licensure with the ND EMS Unit by EMS personnel (ND EMR) who are NOT Nationally Registered. A completed EMS Registration Form must be submitted alongside this completed form. This form may not be submitted prior to January 1 of the year of expiration.
- 2025 Advanced Life Support (ALS) Provider License/Renewal Application Form: This form is for initial/renewal of North Dakota state licensure for ALS personnel and requires a signature from an authorized medical director. Note: as of 2022, this form is TWO (2) pages in length - BOTH pages must be submitted for processing.
