Office Hours

These sessions will discuss upcoming 1915(i) changes related to referrals in Therap, an updated Plan of Care template, as well as the Plan of Care Change Form option. The sessions will offer the same content and provide opportunities for questions.

Care Coordination
- Care Coordination Onboarding Training
- This training is required for all new care coordinators before rendering services
- Care coordinators must watch this entire training, take the quiz at the end, and get a passing score to get a certification of completion. You must print or save this certificate for your records as proof of training completion.
