Coverage to Care, or C2C, is an initiative developed by the Centers for Medicare & Medicaid Services (CMS). CMS oversees how each state provides Medicaid to their residents. The C2C materials below can help you understand your health coverage and connect you to the primary care and preventive services that are right for you.

Requesting Form 1095-B, Health Coverage (Posted Jan. 11, 2024)
Form 1095-B is a tax form that reports the type of health care coverage or health insurance each individual in your household had for the previous year. For the 2023 tax year, the IRS determined that individuals do not need this information to complete and file their tax returns.
North Dakota Health and Human Services will not be mailing 1095-B forms to Medicaid members. Copies are available starting Jan. 16, 2024, upon request through one of the following options:

- Email your request to applyforhelp@nd.gov
