The transition from pediatric to adult health care is a big step for adolescents and young adults and may include new challenges for individuals and their families. Individuals need to learn, practice, and become confident in managing their own health. Families and providers can help support them through this transition.
Moving into adult health care can take several years. There are skills adolescents and young adults can begin to learn that support independence. Adolescents, young adults, families and health care providers who work together as a team can help ensure a smooth transition.
