Conveniently send notifications to 1915(i) staff when members begin working with a new care coordinator and when members stop receiving care coordination services from your agency using the below options.
- New Member Care Coordination replaces the Care Coordination Request Report. Use this form when a member is
- New to your agency and does not yet have a plan of care, or
- Transferring care coordination services to your agency. The new care coordination agency should submit this form.
- Termination of Member Services replaces the Member Discharge Form and should be used to notify 1915(i) program staff when a member is being discharged from care coordination services only without a care coordination provider transfer. When discharging a member for any service, you must still provide written notice to members per the Transferring and Terminating Member Services policy (found on this page).
