1915(i) Individual Eligibility

North Dakota’s 1915(i) will serve individuals meeting each of the following eligibility criteria:

1. age 0+; and
2. currently enrolled in ND Medicaid or Medicaid Expansion; and
3. household income is at or below 150% of the Federal Poverty Level; and
4. WHODAS score of 25 or above; and
5. resides in and will receive services in a setting meeting the federal home and community-based setting requirements, and
6. has one or more qualifying diagnosis
