North Dakota Health and Human Services (HHS) Money Follows the Person (MFP) program stakeholders are meeting Tuesday, Feb. 13, 1-4 p.m. CT at the Southeast Human Service Center in Fargo to review and discuss ongoing efforts to serve North Dakotans with disabilities closer to home and outside of institutional settings.
Since the program’s inception in 2008, the Money Follows the Person program has helped 912 eligible North Dakota Medicaid members with disabilities move from nursing homes, the Life Skills and Transition Center and other facilities to apartments, family members’ homes, or other types of community living arrangements with supportive services from partner agencies. Participation in transition services is voluntary.
The meeting agenda includes discussion on the MFP operational protocol pilot project, the ND transition and diversion services pilot project, housing accessibility issues, the workforce technology project, supplemental services, quality measure set, committee member recruitment, the ND Symposium on Adults, Aging and Disabilities and more.
During the meeting, the HHS Life Skills and Transition Center and Developmental Disabilities Section will share updates on their work supporting individuals in the community.
In addition, the grant’s program reports such as transition benchmarks, housing services, the MFP tribal initiative, Aging and Disability Resource Link (ADRL) transition services, housing task force and more will be discussed.
