  • We work with partners to help older adults and adults with physical disabilities remain living in their own homes and communities.
  • We also act to protect the health, safety, welfare, and rights of residents of long-term care settings and vulnerable adults living in the community.

Family Caregiver Support Program
Federally funded under the Older Americans Act, this program offers help to caregivers.
Lifespan Respite Care (Short Break) Services
Will provide funding for respite services for caregivers of children with special needs, older adults, or other individuals with disabilities in planned, unplanned and emergency situations when caregivers do not have access to other public funding sources.

Long-Term Care Ombudsman Program
People living in nursing homes, assisted living facilities, basic care homes, or hospital swing bed, transitional and sub-acute settings may have concerns about their care. This program helps resolve concerns.

Qualified Service Providers - Independent contractors who care for people in their homes and communities

  • Find a Qualified Service Provider (database) to help care for you or a loved one.
    Information about how an agency or individual can enroll as a Qualified Service Provider (QSP) to provide personal or attendant care services, homemaker services and other approved services to eligible individuals.
