EMS Personnel / Agency Registry (Big Picture) - *Emails were sent to EMS personnel on 9/22/2023 explaining the new login process. Upon your initial access to the new Big Picture system you will need to create a new password. Please follow instructions located HERE. At this time personnel have the ability to update personal information and print their certificate/license. The ability to renew certifications/licensure is not available yet. Please email the EMS office with any issues: dems@nd.gov.
- 2024 EMS Registration: Utilize this form for initial/renewal of North Dakota state licensure, student registration, agency roster updates and changes to demographics. Note: as of 2022, this form is TWO (2) pages in length - BOTH pages must be submitted for processing.
- Non-Nationally Registered EMR, AFAA and EMT Continuing Education Report: This form is to be used by EMS personnel who are NOT Nationally Registered for re-licensure with DEMS and must be submitted along with the EMS Registration Form. This form may not be submitted prior to January 1 of the year of expiration.
- 2024 Advanced Life Support (ALS) Provider License/Renewal Application Form: This form is to be utilized by AEMTs and paramedics for initial/renewal of North Dakota state licensure and requires a signature from an authorized medical director. This form is also to be used by registered nurses seeking to provide ALS for BLS licensed ambulance services. Note: as of 2022, this form is TWO (2) pages in length.
