Platinum Level Long-Term Care Facility Honor Roll Awardees:
Action: Missouri Slope provided antibiotic education to residents on admission and at care conferences. Also, they simplified their antibiotic time out form to improve completion by nurses and providers.
Collaboration: This facility works with providers to improve the nontreatment of asymptomatic bacteriuria. This year they sent providers data to review how the facilities results.
Action: NDHC is utilizing NDHH’s digital brochures to educate residents, staff, and families. They are also utilizing urine culture to drive appropriate antibiotic use to treat UTIs and focusing on appropriate duration of therapy for antibiotic use.
Collaboration: This facility collaborates with Altru for advance lab testing, patient placement, and has interdisciplinary weekly meetings to coordinate patient care between swing bed and long-term care placement.
Gold Level Long-Term Care Facility Honor Roll Awardees:
Action: Elm Crest Manor reviews antibiogram quarterly for appropriate empiric treatment with antibiotics. They also send a letter annually to families/POA explaining what antimicrobial stewardship is, how antibiotics work, when/why they should be prescribed.
Collaboration: They are working with a neighboring rural nursing home on antimicrobial stewardship. They discuss projects, best practices, and policies with each other to improve their stewardship practices.
Action: Marian Manor Healthcare Center provides prescribers monthly report cards on antibiotic use that provide comments on inappropriate antibiotic use if present or if best practices were not followed.
Collaboration: They are working with a neighboring rural nursing home on antimicrobial stewardship. They discuss projects, best practices, and policies with each other to improve their stewardship practices.
Silver Level Long-Term Care Facility Honor Roll Awardees:
Action: Bethany Retirement Living 42nd St Campus provides antimicrobial education to providers several times a year on antibiotic protocol tables and McGeer’s Criteria
Action: Bethany Retirement Living University holds meetings with providers to discuss criteria for UTI testing and provide feedback on unnecessary testing which led to inappropriate antibiotic use.
Action: North Dakota Veterans Home mails out information on vaccinations and antibiotic stewardship annually to residents and families. They also created a form to follow-up on long term antibiotics with providers.
Action: SMP Health Maryhill created a template to recommend certain antibiotics for empiric treatment of uncomplicated cystitis. The template highlights shorter durations of antibiotics for this diagnosis.
Action: SMP Health St. Aloisius has been focused on educating families and residents regarding antimicrobial stewardship and urinary tract infections (UTI). They have used different educational materials (videos, flyers, and graphs) to help increase understanding of what symptoms constitute a UTI.
